This application is for Orthopaedic consultant surgeons or Prospective Fellowship Supervisors wishing to apply for AOA accreditation of a Fellowship Program. To help ensure a smooth application process, please have the following required supporting documentation ready before you begin.
- Updated CV: including research and publications in the last 5 years
- Letter of support: from the primary hospital to demonstrate support for the proposed fellowship program.
- Training Plan/Position Description: with details of the purpose, anticipated duration, locations, content and structure of training.
- Timetable: including the full name of the training sites or a key for the acronyms of the training sites. The timetable is generally in a table format on a weekly, fortnightly, or monthly schedule, including days of the week, shift times, sites and (co)supervisors.
- Certificate of CPD compliance of the preceding year
Applications can be saved at any stage during the process and will be stored for a maximum of three months at which point they will be deleted.
